Food Card and Access Program

HCCI is one of five community-based organizations (CBO) that partner with the United Way of New York City (UWNYC) to pre-screen Harlem residents in targeted neighborhoods who might be eligible for food stamp benefits.

 

Diabetes Awareness

HWS provides community members with diabetes materials that explain the disease, its prevention and treatment, and its disproportionate effect on our community. In past years, HWS reached over 3,000 persons each year through outreach and health education sessions aimed at increasing participants’ knowledge of diabetes.

Immunization

In collaboration with the Columbia University Mailman School of Public Health, the Northern Manhattan Start Right Coalition and funding from the Center for Disease Control, HWS developed a community action plan that endeavors to increase Harlem immunization rates and ensure that all children under 2 years of age receive all of their immunizations for preventable diseases, on time.

 

Health ResourceCenter

The Resource Center is one of HWS’s most frequently used service s. This library houses a multimedia collection of over 300 books, faith sensitive materials, vertical files, academic articles, videos, audiotapes, and CD-ROMs on health related issues. It also offers free Internet access and one-on-one assistance with research.

Asthma Basics for Children

Through a collaboration with the Columbia University Mailman School of Public Health and the Northern Manhattan Perinatal Partnership, HCCI works to identify asthma symptoms among school-aged children, reduce triggers, better manage the disease and reduce adverse health outcomes.

 

Scattered-Site Housing

HWS provides 60 units of Scattered-Site Housing for 15 families and 45 individuals living with HIV or AIDS. Supported by the New York City Human Resources Administration, HCCI provides case management; individual, family and group counseling; support groups, permanency planning; and risk reduction and support.

 

Special Supplemental Nutrition progrom for Women, Infants and Children (WIC)

Twice weekly, the Resource Center signs up qualifying mothers and their children for the Special Supplemental Nutrition Program for Women, Infants, and Children which helps improve the health of those who are at nutritional risk.
